I closed PSP and deleted the category folder located at
C:\Program Files\Corel\Corel PaintShop Pro 2021 (64-bit)\Languages\EN\Print Templates
Then I checked the other 2 file locations PSP uses for print templates.
Nothing is there, except for my 1 custom template file using category "Custom".
Starting up PSP shows the category is still there with just 1 blank template.
But clicking delete on that blank template does nothing.
Not sure what I missed and kind of stumped on how to remove it?

Categories are defined in the print template files, they aren't based on the sub-folder structure. It also appears to cache the categories, though I don't know where that cache may be. Sadly, we can't use the "delete cache" option to force it to reset. But you can use the File Locations dialog to reset it.
In the Print Templates dialog you show above, click the button at the top of the "Category" group to open the "File Locations" dialog. Turn off "Use sub-folders" and it'll re-evaluate all of the folders and files inside. This will force it to remove unused categories. You can then turn "use sub-folders" back on if you want to use sub-folders.

For anyone else who kept using old Jasc psp versions and wants to still manually edit - here's some coles notes
Background:
ie) Old Jasc version 9 would show Print Layout template category's by folder names, and sort by file name, but it would display the description in the edited templates.
--- That allowed for a fast simple way to manually edit templates and let the resulting files be categorically sorted by folder names.
Versus - newer Corel PSP versions Print Layout only pays attention to the info for category and name that's saved in the edited templates.
Location:
File > Preferences > File Locations > then scroll down left-side to Print Templates
This section shows a few Print Template Folders locations
Just look at the bottom "Save To:" for the location to save your custom templates to.
Manual Edit:
There's ways to edit templates in new PSP.

Enough has changed in the top portion its just easier to edit category and description. (highlighted green below)
And for photo templates I increased the value for PrintQuality from 200 to 600. Template section:
To copy and paste the Template section select everything between its square+curly bracket [{ down to bottom curly+square brackets }]
- Deleted all lines starting with EllipticalCell
- Removed the u from 'ImagePath': u'', which now needs to look like 'ImagePath': '',
Hope that helps anyone else batch importing a pile of old templates into the new format.
