And thanks from me, I was not aware of the Blemish Fixer, not that I have had to fix any images like this.
The tool works very well.
I did find that the despeckle options affected the faces causing a soft focus, although was able to replace those with the original faces, several ways to do that.

I will work on it over the next few days and get back to with the results.

If you want a better view of your work, you may create a Contrast/Brightness Adjustment Layer above the image (settings: Brightness 38, Contrast 42). But make your image corrections on the layer underneath.
Another tool you could also apply is Automatic Small Scratch Remover (settings: Lower 0, Upper 3, Strength Normal, Remove dark scratches unchecked, Remove light scratches checked).

I think you mean Salt and Pepper tool (Despeckle works only on single-pixels and no settings).
