Keyword Hierarchy

Under the collections tab I am trying to create hierarchies. For example: Under PLACES I want a sub-category named UNITED STATES which has another sub-category named CALIFORNIA which can have another sub-category called SAN FRANCISCO. etc How does one create such a hierarchy?
Places
United States
California
San Francisco
Golden Gate Park
Of course, under the United States I can 'list' other states.

I would like to do the same with people under sub-categories like family and non-family. eg
People
Family
Jones'
Tom
Dick
Harry
