1. Adjust...Add/Remove Noise...Add Noise. Use "Gaussian", "Monochrome", and 25% coverage.
2. Apply a blur. Use either Adjust...Blur...Gaussian Blur, or Adjust...Softness...Soft Focus. Adjust the parameters to suit.
