Buttons inactive, cannot delete, program crashes

I paid for dvd workshop 2 and have all the patches, updates installed but I'm having big problems with my menu buttons become inactive and I cannot delete them. The relink option does not seem to help. When I copy and paste the menu to a new menu, the buttons are gone, giving me a clue that something is wrong. But what?

Some times when I move the mouse over one of these "stranded" buttons, and right click, the program crashes. Not the best way to handle an error.

This seems to be happening far too often. So, I started the project again, but after a few hours I got the same problem. For some reason different buttons are just becoming inactive. I can't delete them to start over and when I just the FINISH option, they do not exist.

I just don't seem to get it. I'm being very careful, I thought, to make sure all the links in the playlist are correct but this continues to crop up.

And now when I exit the program and attempt to load the same project, I'm getting a strange error stating some menus cannot be found. So, I have to revert to a previously saved project file. But soon, the same problem crops up again. I am beginning to wonder why there is no good error checking here, or at least some better report as to what might happen. The menu system is getting complex but it is not that complex. This should not be crashing just from complexity.

Has anyone else had this same kind of problem and might point to possible issues to avoid?

Also, what is a PlaceHolder? How is it different from a frame, and where to I find placeholders. The PDF file mentions them but kinda falls short on the origin.
