My advice then would be to add each one back one at a time until you find the culprit. My bet would be one of the transitions, especially if it was a "fancy" one.

Have you updated your DirectX
A bit late to the party on this one, but do make sure that you have loaded the latest DirectX update - see the sticky at the top of the forum for details. DirectX is not updated by Windows' Automatic Updates, and when you perform the update, your DirectX version number will not even change, but it can resolve a whole host of problems.
