The Makeover tool is actually three separate tools in one. The Strength setting is not available for the Suntan tool, only for the other two... the Blemish Fixer and the Toothbrush tool.
As you've found, you can't go back over the Suntan tool a second time to increase the effect. That's a good thing, because otherwise the overlaps between brush strokes would show up. You need to do something like close and reopen the image, or duplicate it (Window > Duplicate or Shift + D) to be able to apply a second coat.
The best way would be to duplicate the layer (Layers > Duplicate) after going over it the first time. Then apply the Suntan tool a second time. This second application will only take place on the duplicated layer. The advantage of this is that if you find the second application is too strong, you can reduce the opacity of the top layer in the Layers palette. This will give you a range of suntans anywhere between a single application and a double dose.
A similar procedure can be used if you find a single application is too much. Duplicate the original, untanned layer, add the suntan to the duplicate layer, and reduce it's opacity to suit.
It can also be used if two applications isn't enough... do Layers > Duplicate again, which will now duplicate the layer with two doses of suntan, and apply a third coat.
