I can have a guess:
On a colour block (eg white) on the video track run the painter and paint (scratch) a chroma key colour (bright green will do).
You'll have a video of a white background with a bright green scratch. Use that on the overlay track over the video footage of what you want to appear through the scratch. Use mask attributes to mask out the green scratch. Create the video file
you'll end up with a white block video with the scratch happening and an image comeing through underneath.
No the tricky bit...the video of you scratching over a blue (or something you can mask) sheet of paper on a black table (for eg)
You will now need to use the video track plus 2 overlay tracks. Video track has a colour matching what your hands are resting on.
Overlay track 1 has the white block video we created earlier (in standard version of VS you can no render out this video so that you end up with the face coming through an animated scratch)
Overlay track 2 has you hands going through the motions but we mask out the piece of paper .
That should do the trick.

Yeah I list them here by track , i know most of you are well used to VS but i've tried to describe every step just for new users
Track 1: Background video, girl moving etc
Track 3
Place a large piece of Bright grren cardboard on a table with good lighting
film yourself scratching with a coin, import to computer and add it to track 3 set the Chroma Key settings like in Track 2 and it'll take out all the green background leaving just your hands and the coin.
Track 2:
In the timeline window scrub to where you start to scratch (when you open painter this will be visible in the background)
In painter start recording.
Colour in the whole screen with black
Then "scratch" out lines in a light green colour, about the width of whatever your using coin, brush etc
Stop recording and click ok to go back to video edit mode.
Drag the scratch video into the timeline to track 2 you'll now have an animation of you filling the screen with black and scratching.
Cut out the section of animation where you fill the screen as you don't want that part to be shown.
Select the scratch video and click on Mask and Chroma key
Tick Apply Overlay Options and make sure Type is set to Chroma Key
Then use the Similarity Dropper button to select the colour you want to Mask out (In this case it's light green) you may need to scrub through the video to the part where the green "scratch" is visible so you can select the colour, use the Adjust colour Similarity to get a more accurate mask.
The trickiest part is co-ordinating the scratch with your hand movements.
A very useful addition would be if you could play/scrub through the video clip thats in the background.
