AVCHD : how make own template/profile, and burn disk folder?

I have searched but not seen a simple quick answer:

1. I want to create an AVCHD file, but the Share->Create video file operation only has two "profiles": 1440 and 1920, 18Mbps.

* How do I create an H.264 AVCHD File with a different bitrate?
(eg, how do I create a new profile with various bitrate options to choose from? The "Custom" options here and in Batch convert, only allow MPG1/2, not NTSC-HDMV compression.

I know there is some options in the "gear" option for "Create disk" burning module, but- that creates disks, not files! I want to create files!

2. For burning module then- I want to create AVCHD "harddisk folders", like DVD's "VIDEO_TS" etc, but it only lets me burn a disk itself. The option is greyed out.
How can I do this?
