VS11 - All chapters not previewed when creating chapter menu

VS11 - All chapters not previewed when creating chapter menu
Before burninig to disc, I create all chapters using the Add/Edit Chapters contro. However, when I try to create a chapter menu, not all of the chapters I selected are showing for input into the menu. (Example - 13 chapters created, and only 5 are showing when trying to create the Chaptr menu. I am burning the disc from a VSP Project file. Please advise. Thanks.
