Sources for custom small run full color DVD packaging?

Wondering if anybody has internet links for some place that accepts small runs of full-color DVD packaging. I'd prefer to NOT have a tradional black case with an insert sheet.

I'd rather have a custom look with full-color hard stock paper folded into a 2 or 3 panel.

Also would want a min purchase of 25 or 50 at most.

Disc Makers is a highly respectable company. I think they do small quantities also.
