First install the DeBugMode "Frameserver" plugin. This sets up a new export option named Frameserver in the export dialog. Enter a filename (I use 'Proxy.avi') and save it. VS will go into idle while you proceed to the next step.
Proxy.avi can then be loaded into most any external encoder for encoding, including any H.264 encoders you have laying around. Set up the encoder, set its export filename and start it.
In this case Proxy.avi is a handshake file that facilitates the connection between the timeline and the encoder, pulling frames from the VS timeline in batches and passing them to the encoder for processing.
When the last frame is processed VS goes back to edit mode, the Frameserver shuts down and the encoder says "done!"
