Problem with Smart Trim Feature

Smart Trim does not seems to always work. The first few clips seem to work OK (deleting the original checked), but after a few the same clips that you choose to Smart Trim return to the selection window. If you try to Smart Trim them again you get a new set of trimmed file with a new set of numbers. ie: 1st clip = filename0001. 2nd try will leave 0001 in the directory and create a new filename00001. Any ideas?
