First, what program are you using, and if it is a Ulead VideoStudio program, what version?
To import DVD video, are you using the Insert DVD/DVD-R? In the pop-up window you do browse to your DVD drive and click on the Video_TS folder correct? Then open it, and check the Title(s) and Chapter(s) you want to import correct? If so and you still can not import them, can you describe what happens?
Your question #3, No you should not mix PAL and NTSC in the same project. The reason is, that PAL uses the file spec of 720 x 576 with a frame rate of 25fps, while NTSC uses 720 x 480 with a frame rate of 29.97 fps. I'm afraid you would get alot of jerkiness in your video, if it would play at all..

And finally (we hope), per your original post, the fact that the DVD's are PAL or NTSC is irrelevant to the computer and the import process.
Inconsistent importing will be due to incompatabilities with the media used or the way in which the DVD was produced in the first place. To increase you chances make sure the firmware of your DVD burner is up to date. Also some DVDs are copy protected and are not permitted by law to be copied etc. So it can depend on many factors. You don't say where the DVDs are coming from.
