Label@once & Memorex CD&DVD 3 in 1 Labels

I have the Memorex Part #5395 6634 CD & DVD labels and want to use them with Label@Once. How do I get the template for these labels?

Are they not standard size DVD discs?
There are several templates built into label@once just select them from the drop down boxes on the left under the General tab.
