sound and video synchronisation

The issue of sound and video synchronisation seems to be a recurring question so for the benefit of myself and others dose anybody have any knowledge on how to avoid such problems and what is it that courses it to happen.

Workflow from capture to edit to author...

This is caused by a number of possibilities:

1) captured audio is not "locked" with the video
2) editing video and output causes a "drift"
3) demux/remux of video and audio causes OOS
4) etc...

It vaires depending on computer setup, and process for capture/edit/authoring...

Some things to minimize out-of-sync:
1) use several "shorter" Videos (instead of 1 60-min video, break it into 3 20-min videos
2) if capturing DV .avi, use Type-1 instead of Type-2
3) avoid multiple compressions of the same audio
4) use elementary streams (separate video and audio streams)
